General Warnings
Premedication
Before starting the treatment, premedication with oral folic acid and intramuscular vitamin B12 is recommended to reduce the severity of hematologic and gastrointestinal toxicity.
Blood cell count
This medicine is known to decrease the neutrophils and platelets count. Blood reports should be obtained before initiating the treatment. The dose of this medicine should be reduced based on the absolute neutrophil count and platelet count.
Skin reactions
This medicine may cause severe skin reactions. To reduce the severity of these reactions, corticosteroids like dexamethasone should be given before, during and after [post-title] administration.
Infections
The patients receiving this medicine have a higher chance of developing infections. Hold the chemotherapy and start with broad-spectrum antibiotics if there are any signs and symptoms of fever. Close monitoring of the body temperature and the blood cell count is necessary.

How to use Alimta Injection 500 mg 1 Vial ^
Administered as an IV infusion over 10 min. Pemetrexed is compatible with standard polyvinyl chloride (PVC) administration sets and IV solution bags.
Reconstitution: Reconstitute each 100 mg vial with 4.2 mL of 0.9% NaCl (preservative free). Reconstitute 500 mg vials with 20 mL of 0.9% NaCl (preservative free). Reconstitution of either size vial gives a solution containing pemetrexed 25 mg/mL. Gently swirl each vial until the powder is completely dissolved. Further dilution is required. An appropriate quantity of the reconstituted pemetrexed solution must be further diluted into a solution of 0.9% NaCl (preservative free), so that the total volume of solution is 100 mL.
Compatibility: Stable in NS. Incompatible with diluents containing calcium, including Ringer’s lactate injection and Ringer’s injection.
Stability: Store at 25°C (77°F); excursions are permitted between 15°C-30°C (59°F-86°F). Reconstituted and infusion solutions are stable for up to 24 hours following initial reconstitution when stored refrigerated between 2°C-8°C (36°F-46°F) or at 25°C (77°F); excursions are permitted to 15°C-30°C (59°F-86°F). Discard unused portion.
